If you need to screen volunteers or collect additional information as they arrive and sign in, the Sign-in Console Questionnaire will allow you to do just that.
You can configure your system to display up to three custom questions before successfully allowing the volunteer to sign in, whether they use the Sign-in Console or Mobile Sign-in. For some questions, you can determine a response that must be given in order for the volunteer to be allowed to sign in. There is an additional option to prompt volunteers to accept your Terms & Conditions if they haven’t already done so. For more information about this option, refer to the Knowledge Base article on Enabling Your Terms & Conditions - An Onsite Electronic Waiver. Volunteers will be prompted with this questionnaire once per day, even if they sign in or out for multiple shifts. Volunteer responses will be time stamped and stored as a note on their profile.
Locate the Sign-in Console Questionnaire configuration fields by following the steps below:
- Login to your CERVIS account.
- Click on Edit System Configuration under the “System Management” heading on the Main Dashboard.
- Click on the Sign-in Console / Kiosk Options to expand the section.
From here, create your questionnaire using the fields labeled “Sign-in Console Custom Question” as outlined below.
Sign-in Console Custom Question Type
This field has 3 corresponding selections to make, as visualized below:
If the first selection box is set to the default setting of “Disabled,” the system will not be using this field. To enable it and determine the format of your volunteers’ responses, use one of the options listed below:
- Selection Box - this option will allow your volunteers to choose one item from a list of predetermined responses that you set in the Sign-in Console Custom Question Options field.
- Text Entry - this option will allow your volunteers to type in up to 300 characters with no format restrictions.
- Date - this option will require your volunteers to enter a date.
- E-mail - this option will require your volunteers to enter an email address.
- Phone - this option will require your volunteers to enter a phone number.
- Number - this option will require your volunteers to enter a number.
The second selection box allows you to choose whether this question will be “Optional” or “Required” when volunteers sign in.
The final selection box allows you to determine where this question will be listed. Whether it will be on both the Sign-in Console and Mobile Sign-in when a volunteer goes to sign in, or just one or the other.
Sign-in Console Custom Question Text:
In this field, enter the question you are asking or give a description of the information you want to collect.
Sign-in Console Custom Question Options:
In this field you will create the response options volunteers will have to choose from if this is a “Selection Box” type question. Enter the different options in a comma separated list. For example, if it is a yes or no question, you will enter “Yes,No” into the space provided.
Sign-in Console Custom Question Required Answer (Optional):
If you are asking a Selection Box type question, you can choose one of the options you listed in the field above to be a required answer by entering that option exactly. This will make it so that volunteers must select this answer in order to be allowed to sign in. If they do not select the required response, they will not be able to complete the sign-in process and the “Sign-in Console Custom Question Failure Message” mentioned below will be displayed to them on the screen.

This is the message that will be displayed to the volunteer if they select an answer other than the required answer you defined. Feel free to modify this field to fit your needs. The default text provided is below:

Sign-in Console Verification Method
Finally, we recommend enabling the Sign-in Console Verification Method if you haven’t already. By setting this field to “Phone Number”, it will prompt volunteers to enter the last 4 digits of their phone number (either primary or alternate) in order to verify their identity. This serves the dual purpose of ensuring volunteers are present themselves and that they did not click on the wrong name.
