Question:
How do I upload a medical release or liability waiver for my volunteers to download and fill out?
Answer:
If your organization already has the form(s) on your organization's website or they are hosted elsewhere on a third party online storage system like Dropbox, please follow the steps below:
- Login to your CERVIS account.
- Click on the Edit System Configuration link under the "System Management'' section of the Main Dashboard. Please note, this option will only be available to CERVIS users with the Administrator access level.
- Locate the Information Collection section heading and click on it to expand the section.
- Set the "Track Liability Waiver/Medical Release" field to ‘Yes’.
- Optionally modify or replace the text found in the "Track Liability Waiver/Medical Release in CERVIS as" field to read how you refer to your waiver.
- Update the "Liability Waiver / Medical Release" fields with the complete web site URL (include http:// or https://) for the document(s). If your organization uses the same form for both adults and minors, you only need to input the URL into the adult field.
- Please note if you have a separate waiver for minors, you must have the “Track Volunteer Birth Date” field enabled in the Information Collection section so the system can determine which form they should receive.
- Click the Update Configuration button to save the configuration.
If your organization does not have the forms on your organization's website, then you will want to use the File Management tool. To use this option, please follow these steps:
- Login to your CERVIS account.
- Click on the File Management link under the "System Management" section of the Main Dashboard.
- Click on the Choose File button and select the file that you want to upload from your computer.
- Click on the Upload New System File Attachment button.
- Your uploaded file will now appear below under the "Files Currently Managed for <organization>" heading.
- Click on the link icon () and copy the secure link provided.
- Click the Close button.
- Click the Edit System Configuration link under the "System Management" section of the Main Dashboard. Please note, this option will only be available to CERVIS users with the Administrator access level.
- Locate the Information Collection section heading and click on it to expand the section.
- Set the Track Liability Waiver/Medical Release field to ‘Yes’.
- Optionally modify or replace the text found in the "Track Liability Waiver/Medical Release in CERVIS as" field to read how you refer to your waiver.
- Paste the link you copied into the "Liability Waiver / Medical Release" fields (include http:// or https://) for the document(s). If your organization uses the same form for both adults and children, you only need to input the URL into the adult field.
- Please note if you have a separate waiver for minors, you must have the “Track Volunteer Birth Date” field enabled in the Information Collection section so the system can determine which form they should receive.
- Click the Update Configuration button to save the configuration.
PLEASE NOTE: In order for the system to email a copy of the Liability Waiver/Medical Release to volunteers for completion when they register for an event, the event must be created or updated to show that it requires the waiver/release.
