Customizable volunteer application for specific events

We have service project that we need volunteers to fill out a volunteer application with very specific questions that are not relevant to our other events or service projects. This SP is a long term mentoring project with youth that have very specific grant requirements in the information that we get from the volunteers. Rather than have the volunteers create an online profile, apply through CERVIS for the project, and then complete an paper application, we are looking for ways to streamline the process for the volunteers and staff. We need the ability to create an online application for specific events- so we can either choose between the general application or create a event a specific one. It would also be great if the applications could be downloaded into either excel or word so we can easily share the applicants responses with program staff. I've attached the application from 2014 to get an idea of the questions that we need to ask.
